Vaccination and Other Requirements for CHS Students
All CHS students must submit the required immunization documentation through a third-party vendor prior to the start of classes. Students enrolled in the BSN program will have additional immunization and skill certificate requirements that must be uploaded into the third-party system. Failure to do so will prevent participation in required clinical training necessary to complete the degree.
The following vaccinations are required for all CHS students prior to the first day of class:
- Varicella (Chicken Pox)
- Tuberculosis
- Tetanus, Diphtheria, & Pertussis (TDAP)
- Meningococcal Vaccine
- Measles, Mumps & Rubella (MMR)
- Hepatitis B
- COVID-19 Vaccination (MANDATORY FOR BSN STUDENTS, waiver permitted ONLY for Non-BSN students)
Health Insurance Coverage
All CHS students must provide proof of health insurance coverage.
The following vaccinations and certificates are required for BSN students:
- Prior to the first day of class (Term 1):
- Technical Standards Form
- Health Insurance Card
- Prior to the first day of class (Term 4):
- HIPPA Certificate
- Drug Screening
- Background Check
- Further vaccinations as required by clinical partners
- Prior to the first day of class (Term 5):
- American Heart Association BLS CPR Card
